It's not a side under serve, but a float/no-spin serve. Joshida's flick clips the net probably because he tried to flick the ball aggressively while keeping the return as low as possible and did not quite compensate for the lack of spin on the float serve.

Ouch, not good. I was diagnozed with this when I was 12 years old. Spent two weeks in a hospital with my knees in a cast, then had stop any physical activity like sports (including quitting TT training) or running, jumping for a year. Go see a doctor.
