I think that Benedikt Duda might have the cleanest forehand serves I have ever seen at international, national or even good local player level.

It is my opinion that the rules are supposed to be interpreted in the way he serves. He throws the ball up and gets his arm well out of the way early, it would not matter where you stood you would clearly see the ball at all times!!

Very much so, he has a high toss reverse pendulum serve, that is like a penguin at the heart of the equator I tell you!

No one mentioned Kanak Jha for cleanest serve?
High loss isn't automatically clean, check Hugo if you doubt this. Kanak may be clean but the case stiok has ro be made. Some of his lower toss serves can be dubious.

All of Ovtcharov's serves look clean
For sure the contact point is always visible for his serves, but verticality of his tomahawk serve is not so great. However, players rarely complain about verticality of opponent toss, the contact point is apparently only thing they care (unless they want to play mind games or get opponent TTRed)
